Program Description

Open Heart Community Care Services, Inc. (OHCCS) mission is to provide quality senior home care, residential care, assisting living, and clinical adult day care for individuals that are mentally challenged, disabled or that have experienced traumatic brain injuries.

OHCCS serves individuals and their families with an open heart, changing one life at a time, by working diligently with the health community to sustain or improve an individual’s quality of life.
